GRK4 A486V (rs1801058) is a functional missense variant in the membrane-targeting region of GRK4. The 486V variant (T allele, genomic reference) enhances receptor phosphorylation and desensitization, contributing to salt-sensitive hypertension. In the INVEST trial (1,460 patients), 486V homozygotes had significantly increased risk for adverse cardiovascular outcomes (OR 2.29, p=0.0002).
